'it' cannot be antecedent (describe) of the clause. 'it' can only be antecedent of the single noun.
A,B,C are out.
Choice E alternates meaning - 'although' is different from 'but'
Correct answer is D.

Clause --> Chinese scholars accurately described the flow of blood as a continuous circle controlled by the heart, but it(incorrect antecedent)
