D in a European city will usually be five times more efficient than will a system serving a comparable population in an American city

E that is implemented in a European city will usually be five times more efficient than if it is implemented with a comparable population in an American city

A noun modifier should be close to the noun it modifies. So a commuter rail system should be followed by "in a European city". Only D is the potential answer.

A. His campaign for sanitary conditions in operating rooms finally successful

E. Campaigning, eventually successfully, for conditions to be sanitary in operating rooms
Good question. The original one is correct

Because and since cannot be used here - its not incorrect. But I wont prefer it - as the original sentence do not provide such reasoning.
In D -- "finally successful" is converted to "being eventually successful" : we are not allowed to do that. The essence of the original sentence should be maintained.

So its A. This is an absolute modifier here - "His campaign for sanitary conditions in operating rooms finally successful" .
