(one in ten) is just a ratio. Total no. of ppl admitting to join the gym depends on the total number of people. It is generally plural, unless the total number of people is 10.

Also it's not a collective noun like an army,committee. Hence Plural can be used.
"The committee members admit to" - plural used
"the committee admits its limitations" - singular

IMO Plural

PS: Pls correct me if i'm wrong

Which of the following is correct?

Exactly one in ten health club members admit/admits to joining a gym for social reasons.

But here admit refers to exactly one, which is singular.

IMO it should be

Exactly one in ten health club members admits to ....
