A: Just because King Alfred occupied and fortified London...did not mean
Here, the portion in red is a MODIFIER and thus cannot serve as the subject of did not mean.
Eliminate A.

C: Just because King Alfred occupied and fortified London, it did not mean
E: Just because King Alfred had occupied and fortified London, it did not mean
In each of these options, it seems to refer to the MODIFIER in red.
A modifier cannot serve as the referent for a pronoun.
Eliminate C and E.

In D, the fact (subject) lacks a verb.
Eliminate D.

The correct answer is B.
