How far apart should practice tests be spaced out?

I would take 3 total practice tests. I would say every other week is a good frequency. Taking more than that will probably be counterproductive in my opinion. After 3 practice tests, I think it's best to spend time working on your weaknesses.

When should I take the FIRST practice test? After 2 weeks of when I first begin to study?

When you say three tests, I'm assuming you don't mean this to include a diagnostic test?

This is how I would think.
I'd prefer taking 7-8 practice tests for best results (includes 4 GMAT Preps)
You can't go wrong even if you equally space them out in your total duration of preparation. :)
So,
Is your GMAT 3 months away? Then, 90/7 = 13
So in this case, you could space your tests once a fortnight.

Regarding the first test, there are different schools of thought. I believe in taking a diagnostic GMAT Prep in the beginning to best assess where I stand and what my strengths and weaknesses are. Your test results can be analyzed threadbare by a GMAT Prep or even an experienced GMAT taker!
