Bankers require that the financial information presented to them by mortage applicants be complete and follow a prescribed format.

A. be complete and follow a prescribed format
B. is complete and it follows a prescribed format
C. be complete and a prescribed format is followed
D. to be complete and a prescribed format be followed
E. be completed and it followed a prescribed format

IMO A.

B/D - require that + be followed by base form of the verb - this structure is missing.
C - a prescribed format is followed - wrong both verbs need to be parallel
E - it followed a prescribed format - wrong same as above.
Getting defeated is just a temporary notion, giving it up is what makes it permanent.

IMO A

Question tests Subjuntive mood

Require + that + (verb in infinitive form without to)

Left with c and e... both of which dont have parallel construction...
