E - elevated cholesterol levels REQUIRES PLURAL VERB (ARE/CONTRIBUTE) Drop E (IS); Moreover, ITS A FRAGMENT or RUN-ON (ARE at the end; two verbs HAVE NOT DETERMINED and ARE not properly connected)

B - they have not determined the causes of them or how cholesterol affects the heart
parts in BOLD are not ||lel; THEY...THEM (inconsistent pronouns)

C - THEY...THEM (inconsistent pronouns); NEITHER...OR (idiom problem)

D - neither determined their causes nor how cholesterol affects the heart
parts in BOLD are not ||lel

IMO A

nutritionists ............contribute to heart diseases so negate D and E,

left with ABC....
in B and C,.,. 'them' is used which we might take for elevated cholestrol levels but we got same possibility to treat them for nutritionists so negate B and C..
left with A, which removes ambiguity by mentioning elevated levels later on...
