How is it [spoiler]C with "HAD BEEN"[/spoiler]
That is the correct usage. If you look at the meaning of the sentence than you will understand that the dogs were first collected to protect local residents from dogs attacks and then they [dogs] were distributed to local pet sanctuaries. Tense issue is solved in this option

This question is similar to an OG question on Earthquakes. If you chose A, it is wrong because it suggests that the dogs were distributed and exterminated at the same time. This makes no sense

In C, there are two past tense events.
City had taken an initiative to protect local residents -- This occurred before
The dogs were either distributed to the local pet sanctuary or were exterminated -- This occurred later
