For decades Surgeon Generals have warned that smoking increases the risk of lung diseases, including lung cancer and emphysema, and can complicate pregnancy, and increases the risk of low birth weight.
(A) can complicate pregnancy, and increases the risk of low birth weight
(B) can complicate pregnancy, and increases low birth weight risks
(C) can complicate pregnancy, increasing the risks of low birth weight
(D) can complicate pregnancy by increasing the risks of low birth weight
(E) complicating pregnancy by increasing the risks of low birth weight

Generals have warned that smoking increases the risk of X, including A and B, and can + Verb in base form + Preposition

=> The last and in the options A and B is not required since there is an AND in the non-underlined portion.

=> Can + Verb in the base form--is the correct grammar. So, E is out.

Comma + VerbING: in the option C is NOT correct because "COMMA + VerbING" modifies the previous "NOUN + VERB".

Answer should be D.

but when we use "by" - it seems to say that increasing the risk of low birth rate can complicate pregnancy. can you please explain?

Its not low-birth rate. It is low-birth weight. I think this should clear it all.
