A VERBing can function as a NOUN or as a MODIFIER.
A VERBing that functions as a noun is called a gerund.

D: the act of stinging causing the bee to sustain a fatal injury
Here, causing is not a gerund but a MODIFIER serving to describe the act.
Question: What KIND of act?
Answer: the act CAUSING the bee to sustain a fatal injury.

Mitch sir,why prep+noun+ing is incorrect ,can you please explain this in layman term? I Have searched various forums but still not able to get a satisfactory answer.

PREPOSITION + NOUN + VERBing is a valid construction.
SC26 in the OG12:
over a period beginning a few years before Susan's marriage
Here, the portion in red = PREPOSITION (over) + NOUN (a period) + VERBing (beginning).

In many cases, however, PREPOSITION + NOUN + VERBing will distort the intended meaning and thus will be incorrect.
In answer choice D, this construction is invalid because it distorts the intended meaning.

what does 'with the result that the..' modify? which sentence?

As far as I know 'with...' modifer answer question 'how does the action happened'. I see nonsensical to say:

How does stinger stay?

it stays with that the act of stinging causes.

Generally, COMMA + with the result that serves to introduce the RESULT of the preceding clause.
The stinger stays where it is inserted, with the result that the act of stinging causes the bee to sustain a fatal injury.
Here, the portion in red is the result of the preceding clause in blue.
