Can someone please explain what actually is "to be" form of verb?

I'm no expert, just trying to work on my skills. If I've made any mistakes please bear with me.

I am not exactly sure what question you are asking.

'To be' is in the infinitive form.

I was actually looking for verb form "to be"(is,are,am,etc.). Nevermind, I got what I was looking for.
