B is correct.
These CR questions that you have been posting recently are pretty flawed.

As for option B, it says that the number of cigarettes per person is half of what is smoked in America.
This means people are inhaling less smoke in Japan as compared to the people in America.
Hence there is low rate of cancer in Japan.
Correct Option should be B.
Option D just says that Cigarettes in America are of better quality. The quality does not directly mean that they would give out less smoke. Too many assumptions here.
