D is incorrect because the reference of all the stars together representing ONe of the five titles is incorrect.
E is incorrect because of the usage of the Pronoun It's. It's does not directly refer to the Club and the usage is hence ambiguous.

Hi j_shreyans,

The correct answer is A.

The reason D is incorrect has to do with "all" and "one," shown in red below:

The jersey of the Colombian soccer club Independiente Medellín is emblazoned with five stars, all of which represent one of the club's five national titles.

It doesn't make sense for "all" five stars to represent one of the club's five national titles. If there are five stars, and each star represents five national titles, then they must have won 25 titles. OR we should only have 1/5 of a star on the flag to represent a total of five wins.

The reason E is incorrect is the confusion between the pronouns "them" (plural) and "its" (singular), noted in red below:
