A - In assessing the problems, ____ who did that ? No answer could be found in A. There is nothing wrong with the voice but its a opening modifier problem, we couldn't find a subject in it.
B/E - 'they' lacks a clear referrant
C - Seems to suggest as if the question is assessing and not the people

IMO D.
Getting defeated is just a temporary notion, giving it up is what makes it permanent.
