Because is usually found in an adverb clause of cause and effect, i.e. Jacob fell because he is clumsy. or Because he is clumsy, Jacob fell

Caused by has the same essential meaning, but is used differently. It's usually used with a linking verb for a passive construction , i.e. Jacob's injury was caused by his clumsiness.
