The correct verb form is plural "are." The verb refers to "teens."
Consider the following examples:
Forty percent of the students are in favor of changing the policy.
Forty percent of the student body is in favor of changing the policy.

Quite often schools want the student to have a score not older than 2 years - however, this is quite an individual thing. That's why the best idea is to contact the targeted school and ask what their preference is.

I think it would be a better idea to take the GMAT closer to your application date - at best 1 year before the application or in the same year. It would also be a good idea to inquire directly at the school you want to apply to what recommendations they can make in this respect.
