The use of the plural form "populations" does slightly change the meaning but this change is just a minor flaw and not a serious mistake. The plural form "populations" may be referring to a few populations of herons living along the Maine coast.

The correct verb form is plural "are." The verb refers to "teens."
Consider the following examples:
Forty percent of the students are in favor of changing the policy.
Forty percent of the student body is in favor of changing the policy.

Quite often schools want the student to have a score not older than 2 years - however, this is quite an individual thing. That's why the best idea is to contact the targeted school and ask what their preference is.

Answer A is incorrect because the clause that follows "the efforts of investigators" should not be an essential clause. This clause is not essential to the meaning of the sentence. Therefore, answer B is the best choice.

I think it would be a better idea to take the GMAT closer to your application date - at best 1 year before the application or in the same year. It would also be a good idea to inquire directly at the school you want to apply to what recommendations they can make in this respect.
