IMO D. I really don't know why D is incorrect.

I can point out following problems:
1. as vs when: aging is a gradual process. 'as people age, their concerns change as well' is correct. Eliminates C and D.
2. Subject-verb agreement. Eliminate A.

I am confused between B and E. Going for B because it sounds better to me, but I don't have any explanation for it.

IMO B.

Choices A and C can be eliminated very easily due to Pronoun Incongruity. A person...their does not make any sesnse.

For ruling out D the logic has been provided very aptly.

Between B and E, B is certainly a better choice. As E doesn't follow Parallelism. Realizing is a gerund, no other part of the non-underlined segment uses a gerund.

Going by the sounding logic, you many goof-up with the questions :)

"The realization" is parallel to "Legacy",
"To realize" is NOT parallel to "Legacy";

Do not use "when" to describe prolonged process, use "as".

I think that they test here the following rule:
When you use "BE" to equate or compare two elements, the elements MUST be parallel:
- Vanilla IS more popular than chocolate;
- Seeing IS believing.

In our case:
- "One legacy ... IS the realization ... " - Good paralellism: noun=noun
- "One legacy ... IS to realize ... " - Bad paralellism noun is compared to infinitive.
