Because the financial review covered only the last few fiscal years, and therefore the investigators were unable to determine the extent to possible earlier overpayments.
(A) and therefore the investigators were unable to determine the extent to possible
(B) so therefore the investigators were not capable of determining the possible extent of
(C) therefore the investigators were unable to determine the possible extent of
(D) the investigators were not capable of determining the possible extent of
(E) the investigators were unable to determine the extent of possible

why the choice is not D?

This one is slightly tricky... it's got to be either D or E, but which one do we pick? Well, choice E is less wordy (not capable can be supplanted with unable). So the answer is D.

I think "D" changes the meaning by flipping "possible" with "extent".

I see two errors with D .
1)not capable of changes the meaning of the sentence also it is lengthy. unable is the right term here.
2)In "possible extent" possible modifies the extent not the earlier over payments.

Ah, I'm sorry.. I had a typo in my post. I meant to state that the answer was E. Answer D changes the meaning.
