When Sam allows for a 16 m head start he and john reach at the same time, so Sam requires the 4 seconds to cover this additional 16m.
Hence, speed of Sam = 16/4 = 4 m/s.

The triangle specified here is a right angled triangle (9^2 + 40^2 = 41^2).
Always remember that in a right triangle, the hypotenuse is the diameter of the circumcircle and the centre is exactly the mid point of the hypotenuse.

A small trick here.
2. basically says that y=0. But it does not say anything about x right. If x were to be zero as well, then the equality would hold. So, we cannot say for sure based on 2 alone.
Using both the statements, we can say for sure that the equality will not hold, hence C is the answer.
