I think, "who" doesn't refer to all the persons who bake special cakes, its referring to only one of them ("He"). So the verb will be singular when you're using "one of the"."He is one of the persons who bakes special cakes."

IMO 'D'.
(A) Not sure if 'In assessing' is ok.
(B) 'is / they' is a mismatch.
(C) 'A question that is irrelevant' and 'face is whether' make the sentence complicated.
(E) 'rural migrant worker is better' / 'that they face' is a mismatch.

I think the reason is
'....listeriosis, afflicts some people in a contaminated area though it spares many others. '
The subject reference of 'it' is not very clear, so A and B can be ruled out. Among C, D and E, the most appropriate option would be 'E'.
