I also plan to take the GMAT around the third week of December. Though will be a bit stretched to meet the deadlines but I really want to score well and I don't think that I can do well right now so you shouldn't be late i think

Sometimes people do well on the actual GMAT inspite of poor performance on practice tests but the word here is "sometimes" i.e. it doesn't happen often. I'd rather take the GMAT a little later than end up taking the test on the hope that a miracle will happen

As long as you get a 5-6 on IR I think it shouldn't be a problem at least this year. From next year, a low score can be an issue though.
I agree with David's first point though.

With respect to your question no. 2, you have to select five schools where you will be able to send your GMAT score free of cost. You will be able to send your scores later also(anytime in the 5 year window from the time you take the test) but you will be charged around $29 for sending per school.
