Recommender Form Question Advise

Hi

I am working on completing my applications for the 2011-'12, but I have run into a slight dilemma. When I solicited recommendations, I was under the assumption that the recommender would just upload their word recommendation and be done with it. My supervisor, for example, worked after hours to finish my letter and informed me when she was completed so I sent her the recommender form to submit her form and she called me to her office to and said "what is this" The application required her to answer specific questions. .....I am not sure how I would have/should have know this when going through the application process. Would it be viewed unfavorably if she were just to copy and paste her recommendation and leave the questions unanswered?
I would be a huge inconveinance to have her complete a form as she is super busy and has already taken the time out to complete a letter of recommendation.

Please advise

Hi King Tmo,

It sounds like you should have given her the recommender form when you asked her to write the letter of recommendation. Since the school is asking specific questions about your candidacy, it would be most helpful if she did specifically address those issues.

I'm surprised at your trepidation to approach her about addressing the additional questions. Is it because you feel like you dropped the ball--or because she is making it seem like a huge inconvenience to write the recommendation? If it's the latter, I fear that she may not be an enthusiastic recommender for you.

Relax, King Tmo! Once you have made your recommender packet, you shouldn't have too many more troubles.
