How to select B-school to send GMAT score before the exam?

Hi,

I have my GMAT exam scheduled on March 02, 2013. As of now, I am not confident of getting a 650+ score . Since, I have already rescheduled it once and I am better prepared now than I was before, I am thinking of giving it my best shot with current preparation level. My score on veritasprep free online test was 610.

I need this forums help in deciding on the below two conditions:
1) Do I have to mention the program name along with the B-school name where the GMAT score will be sent to?
2) If I fail to get the target score , but would have already selected B-schools with average score of 650+ or 700+, will it negatively impact my application incase I have to retake the GMAT in future.

Your valuable inputs will be really helpful
