How can I prove I have extracurricular activity?

I have the join some extracurricular activities when I am undergraduate student, but these never show in my transcripts. Can I use the certificate of take part in sport competition as evidence that I am a member in a sport team? Another problem is the Student Club of volunteer work, I have joined this club, but I have not received document from them, so how can I prove that?

"Proof" of extracurricular activities is not required. Usually applicants just list them and provide a brief description and that's that. Call it the Honor System.
