Include essay prompts in responses?

This is a silly question. When submitting an essay response as a word doc or PDF, should I include their question at the top of the page to make it clear what I'm answering? Should this could toward the word count?
Practice Tests 720, 720, 680 750, 770, 750

Real GMAT 1 - 730 (Q46, V45, AWA 6.0)
Real GMAT 2 - 750 (Q45, V48, AWA 6.0)

How do you format your essay? Lets say the essay has three parts, and they give you the question in the form of:

The main idea of the question...
a) Detail 1 they want you to answer
b) Detail 2
c) Detail 3

Do you use precious space to include the prompt? Do you separate the sections of your response into a) b) and c)? Or do you just try to address all of it in your essay, but keep it in paragraph form without delineations between the sections?

Thanks for your advice on a silly but technical question!
