Simple SC gone wrong

Usually appearing as dirty water, sludge is formed in the ocean from biosolids and chemical waste, the two primary waste products of industrial emissions, react with ocean water.

a) sludge is formed in the ocean from
b) sludge is formed in the ocean when
c) sludge, formed in the ocean when
d) sludge is formed in the ocean, and when
e) sludge, formed in the ocean from

a) sludge is formed in the ocean from (creates the incorrect construction "from biosolids and chemical waste...react with ocean water")
b) sludge is formed in the ocean when (works; "when biosolids and waste...react with ocean water")
c) sludge, formed in the ocean when (fragment; we don't have a verb for the subject "sludge")
d) sludge is formed in the ocean, and when (makes it sound like there are two different causes for sludge)
e) sludge, formed in the ocean from (same fragment problem as C)
