The proposed health care bill would increase government regulation of health insurance, establish standards that would guarantee wider access to people with past health problems and to workers changing jobs who otherwise could be uncovered for months.

the sentence means: the proposed bill would increase govt. regulation by establishing certain standards.

A. seems like increasing regulation and establishing standards are two separate functions. also "who" seems to modify jobs rather than people

B. correct and logical=> the bill proposes to increase regulation (by) establishing standards. also people who are changing...and (who) otherwise ....is parallel.

C. gets it backwards. bill proposes to increase regulation to establish standards as though establishing standards is the main purpose
also says "that would be uncovered" modifies jobs instead of people

D. gets it backwards like in C. "who" modifies jobs instead of people

E. gets it backwards like in C and D. also "that would be uncovered" modifies jobs instead of people

hence, B

Effect: establishing standards

Thus "establishing standards" will modify cause.....

IMO, B

C and E can be eliminated for the modifier "that otherwise could be uncovered" modifies jobs, hence unlogical.
No pain, no gain.

IMO B . this is a modifier thing

Participle phrase would correctly modify the health care bill. Hence B

"D" IS WRONG, COZ "access for" IS UNIDIOMATIC,
"ACCESS TO" IS THE CORRECT IDIOM.
