Test grid designed to mimic test checkboxes

I created the attached OG 12th edition Checkbox Test Grid report to quickly and efficiently identify problem areas. The report uses checkboxes to record your answers and the button on the "Problem Report" page pulls only the questions that were answered incorrectly for careful review. The advanced filter works off the correct/incorrect checkbox fields, so this is the only field you really need to fill in for each question - the rest of the metrics are to help identify issues.

There is a command button called the "PDA Report" which copies the report into a smaller PDA friendly version. The PDA version helped me review questions I missed when I didn't have access to my laptop.

Important: The report needs to have macros enabled and it is designed for Excel 2007 and later versions.
