After several years of rapid growth, the healthy care company became one of the largest health care providers in the metropolitan area, while it then proved unable to handle the increase in business, falling months behind in its payment to doctors and hospitals.

If we look at the meaning of the sentence we can identify two opposite situations- first the company experienced growth and then something else (opposite) happened. Therefore, the two clauses should be split by an opposite conjunction, in our case BUT. For that reason, we can eliminate A and B.

So we are left with C, D and E.

D is incorrect because of the tense. All actions should be in past simple here. C and E both introduce 'past tense' but C is idiomatically incorrect due to 'in its paying'.

Therefore, IMO the correct answer is E.
