Congrats on the score improvement, dav35! Sorry to hear about your illness though, but seems to me that you managed to overcome that. I don't think you need to retake - I only see that as being necessary if you're part of an overrepresented group of applicants. Best of luck for fall admissions!

They may support it, but to me most of the time that's not necessarily the case. Those three options are meant to be the summary of the text, not just supporting ideas for the one bolded sentence.

Writing tips/templates
How about a combination of both? Although in general I'm not a big fan of citations, it's really not worth learning these to insert in essays and most of the time they don't actually add value. Also, beating about the bush with general stuff won't score you too many points... You need to be concise ...

Most of the time yes, but it also depends on what the actual question is asking. When you write a summary, the first sentence is usually the thesis, but it really depends: this introductory sentence might just be a starting point and the conclusion or thesis might come later.
