overall your score is good enough to be accepted into any program. Now that doesn't mean you are a shoe in or will be accepted. You need to work on your application essay's. Get great recommendation letters, and do well when you go in for your interview. Also I it depends on your work experience too...

Yes UCLA offers the Fully Employed MBA program called FEMBA. It's nationally ranked number 1 for part-time MBA programs.

Congratulations on your Gmat! Unless you want to teach at top University it really doesn't matter where you get your Phd from.
Work on other parts on your application and you should be just fine! Also, already having an MBA can give you a leg up against the competition.
