All top schools will only accept GMAT scores sent directly from GMAC. There's no exception to this (which is unfortunate, because GMAC of course charges you to send the score reports).

It probably depends on each school, but most recommenders will get login details to a website where they then submit the recommendation letters.

I'd agree that if you took the GMAT once you should only go for another try if you think you can up that verbal score. Also, post-MBA career goals matter.
