sine the underlined portion starts with "and", that part of the sentence must be the end of a list. The trick to solving this question is understanding where the list begins. The answer is E. I go through the question in detail in the full solution below (taken from the GMATFix App).

This SC can be solved by using a couple of different grammar rules. Based on its construction, I'll handle it with a Modification rule....
The opening part of the sentence tells us "The diet of the....Greek....was largely vegetarian...." The next part of the sentence defines what a vegetarian diet is/was (thus the modification), so it can essentially be ignored. The last part of the sentence provides contrast to what was rarely in a vegetarian diet: meat. We need a phrase that properly describes meat and its rarity in the diet.
Final Answer: E

The first and signals the END of the list of vegetarian foods.
Since meat is not a vegetarian food, it should NOT be included in the list.
Thus, the second and is inappropriate.
Eliminate A and B.
C: with meat as rare
The structure here is as + ADJECTiVE.
This structure cannot stand on its own.
Generally, it will be followed by as + NOUN, as in the following cases:
as happy as a clam
as bright as the sun
as rare as a diamond
Eliminate C.
D: vegetables, fresh cheese, oatmeal, and meal cakes, meat
Here, a connector is needed between meal cakes and meat.
Eliminate D.
The correct answer is E.
