CORRECT
B)Concerns about public health have led to the contruction of three separate sewer system between 1876 and 1904 to serve

BOTH A and B sounds good gramatically.. but watch out present perfect form in B. This is wrong as the construction is already over 100 years back.

C)Concerns about public health have led between 1876 and 1904 to the construction of three separate sewer system for serving
Requires lead to form
there should not be any gap between lead -- to
D)There were concerns about public health leading to construction between 1876 and 1904 of three separate sewer system serving

Should be lead to
lead to is better than Leading
E)There were concerns leading between 1876 and 1904 to the construction of three separate sewer system for serving
lead to is better than Leading

C is wrong because of the use of have led to, construction had been done in the past i.e either past perfect or simple past sentence should be used
