Rule: choose words carefully. when you are using strong words or words used in subjunctive construction, OMIT should.

eg: the law requires that the innocent be saved. (correct)
the law requires that the innocent should be saved (incorrect)

Hence A is incorrect. answer should be C

The answer has to be C.
The word 'insist' indicates that Command Subjunctive form should be used for the sentence.
It has to be a command. Hence, the 'author pay' should be used.
