I know you have to wait 31 days before you can retake the GMAT but I was wondering if the same waiting period applies if you simply missed your appointment (i.e. never actually sat for the test). Are there any other penalties associated with this (other than losing $250)?

i know this may be stating the obvious, but why dont you just try booking another session online? If the system doesn't recognize that this was a missed appointment, then call up the GMAT people and explain the situation.

The 31 day rule only applies to people who actually sat it because 31 days is the time it takes for the system to cycle to a new set of questions. So someone can't just sit it on consecutive days and see the same questions.
