While writing the GMAT test, I chose the wrong institution as my undergraduate institution by mistake. The official scores have been generated and it shows the undergraduate institution information incorrect. Is that a problem ? Please let me know what can be done about this. How can I rectify this? Does the admission committee really see just the scores in the report card or other pieces of information are as important as the scores?

This is an innocent error, and I do not think it is a big deal. You may want to call to see if it can be corrected, but if not, the schools will look to your undergraduate transcript for verification on where you attended, not your GMAT report. If the question ever came up, you could easily explain it as an error caused by test day nerves.
