Is "and also"in OG definitely wrong?

Is "and also"in OG definitely wrong?

I am asking this question because I have seen that OG clearly explains that an answer choice is wrong because " and also" is redundant. However, I have also seen the appearance of " and also" in their own words. I mean they write things with this phrase but at the same time say it is redundant in the explanations( which means it is not the right usage?). The same case is for" comma, which". I studied some grammar prep book saying "comma, which" cannot refer to the preceding clause in the GMAT. Is that true?

In addition, for some other OG problems, some choices include " and also' but this time OG says NOTHING about it. Does it mean it is generally ok to use " and also"? I am really confused.

So "and also" is definitely wrong? Whenever you see it appear in the answer choice, you should eliminate that choice? Or it is just redundant but grammatically
right and we should keep it as the right answer if no other better choices appear?

Nah.
Ignore OG's explanation that says and also is redundant.(BTW, you will have to ignore a lot that which official explanations offer, time and again).
However, I have also seen the appearance of " and also" in their own words.
Yup, have a look at the following official SCs' OAs.

Official explanations are confusing and often contradictory. Ignore them.( If in doubt, you can always get it cleared from tutors here on BTG).

Yes, without an exception!

As there is no issue with and also, there will always be much glaring issue(s), or decision point(s), in such situations; for instance, have a look at the following Official SC-
Diesel engines burn as much as 30% less fuel than gasoline engines of comparable size, as well as emitting far less carbon dioxide gas and far fewer of the other gases that have been implicated in global warming.

A. of comparable size, as well as emitting far less carbon dioxide gas and far fewer of the other gases that have
B. of comparable size as well as emit far less carbon dioxide gas and far fewer of the other gas having
C. of comparable size, and also they emit far fewer carbon dioxide and other gases that have

So "and also" is definitely wrong?
No.
Whenever you see it appear in the answer choice, you should eliminate that choice?
No.
Or it is just redundant but grammatically
right and we should keep it as the right answer if no other better choices appear?
Redundancy is a legitimate error in GMAT SC.
and also, however, is not redundant in the eyes of GMAC and thus is a perfectly acceptable construction.
