I went thru it again and am 99% sure its D (Just need to know the OA for the 1%)...let me try to explain...in this question we need to see which verbs are supposed to be parallel . For this we have to understand the meaning of the sentence to figure it out.
The basic thing we need to see is that doctors are not required to spend less time with the patients, but because doctors are required to see more patients, this leads to their spending less time with them! (this is my first explaination on this forum. pardon me if its confusing )
a: 'spend' is ungrammatical here (no logical subject).
b: imposing, requiring, and spending are all parallel, means that insurance plans do all three of these things - obviously plans cant spend less time with patients!
c: all three verbs are parallel again-same absurdity witnessed in choice b. - again, plans cant spend less time with patients!
d (correct IMHO)
e: 'requiring' and 'spending' are parallel in the modifier, implying that the plans themselves spend time with patients. Am sure this doesnt make sense!!
will appreciate if someone can pls post in the OA...

I will go for D ONLY if "and" is not there in ",and spending"...then D would be the best choice.

Ok, i just found googled a bit and found that the question posted is incorrect. (thanks Mission Mba!!!!) There should not be any 'and' (as dilip has mentioned) in the answer choice D. This is a GMATPREP question. And OA id D.
