D. include melting the polar ice caps and sea levels that are rising
E. include melting of the polar ice caps and a rise in sea level
A, B and C: Possible long-term consequences...may include
Here, possible and may are redundant.
Eliminate A, B and C.'

D: Possible long-term consequences...include...sea levels
This meaning is nonsensical.
Sea levels are not a consequence of the "greenhouse effect."
There were sea levels a thousand years ago.
The intended meaning is that a RISE in sea level is a possible consequence of the "greenhouse effect."
Eliminate D.

The correct answer is E.
