Evaluation Grid Advice

I'm writing a recommendation for a few top 5 schools and have been asked to fill out an evaluation grid. Could someone help me with how this should be filled out. The candidate is extremely well qualified but lets be honest no one is perfect. Are there sure fire no-no's in this process?

It has been a while since I evaluated an application (was an adcom at Wharton) but the general trend was for recommenders to be checking the top two boxes along the scale for each of the questions asked, for the strong candidates.
Applicants don't get admitted because all the top boxes are checked, but a "red flag" can be raised because one of the questions receives a below average check etc. cheers, alex
