Bankers require that the financial information presented to them by mortage applicants be complete and follow a prescribed format.
A..
B. is complete and it follows a prescribed format
C. be complete and a prescribed format is followed
D. to be complete and a prescribed format be followed
E. be completed and it followed a prescribed format

why is d incorrect?

Oberline College in Ohio was a renegate institution at its 1833 founding for deciding to accept both men and women as students.
A.
B. for the dicision at its 1833 founding to accept
C. when it was founded in 1833 for its decision to accept
D. in deciding at its founding in 1833 to accept
E. by deciding at its founding in 1833 on the acceptance of

I'd choose A. You need a "be complete" since the clause is introduced
by "that".

The sentence that maintains both parallelism and the correct
verb form seems to be "Bankers require that ... be complete and follow..."

Hmm, not too sure.

I'd rule out A and B as they say "at its 1833 founding" which means
that it was also founded at another time. Founding in 1833 sounds better.

C has "founded in 1883 for its decision" which incorrectly means
that it was founded for its decision.

E "on the acceptance of" looks incorrect.

POE would lead me to D.
