    my answer is E

    Most efforts to combat such mosquito-borne diseases as malaria and dengue have focused on either vaccinating humans or exterminating

    "such as" is idiomatic.

    "have focused" is a proper verb of the noun "efforts"

    on either X or Y , not on either X or on Y. see the difference -"either on X, or on Y"

    E is the right answer.
    Such as should be used instead of like.
    Either X or Y.
    X and Y should be parallel.
