Here is a SC question from Kaplan CAT2 I stumbled upon.
Though initially opposed to the measure, the governor approved the new needle exchange program at the urging of his own doctor, his chief advisers, and a coalition of some twenty social action groups.
1) at the urging of his own doctor, his chief advisers, and a coalition of some
2) as a consequence of having been urged by his own doctor, his chief advisers, and a coalition of some
3) on account of being urged by his own doctor, chief advisers, and a coalition of
4) as he was urged to do by his own doctor, his chief advisers, and a coalition of some
5) as his own doctor was urging him to do, along with his chief advisers, and a coalition of
ANSWER: 1 is correct.
Could somebody educate me on why is 4 incorrect?

avoid passive form of senteces. Choice 4 is passive. Try to pick sentences that are in active form ....
was urged, was pulled, was sentenced, was fired ( wrong since passive )
urging, pulling, setencing, firing ... are active forms with corresponding appropriate subjects. |

aprrove/disapprove of is the correct idiom.

Could you be more specific on when to avoid passive form?
I am guessing there might be situations where passive might be more appropriate.
