I'd say 'B' is wrong because the tense is changing from simple past (had signed) to continuous (while in adolescence). This can still be done, if it was written as 'while she was in adolescence'

Although she had signed a pledge of abstinence while being an adolescent, Frances Willard was 35 years old before she chose to become a temperance activist.
    (A) while being an adolescent
Although and while both are used for contrast clause. So it will be redundent to use while.
    (B) while in adolescence
    Same as A
    (C) at the time of her being adolescent
    being is redundent and awkward
    (D) as being in adolescence
    Same as D
    (E) as an adolescent
    Correct. clear

    Take examples:
While being a kid you were too naughty - is awkard
When you were a kid you were too naughty - is good
As a kid, you were too naughty - is right.
Similar analofgy fits to the question:
Although she had signed a pledge of abstinence while being an adolescent, Frances Willard was 35 years old before she chose to become a temperance activist.
Although she had signed X while being a adolescent => is like saying
She had signed X when she is an adolescent ----> past perfect tense precceds a action in the present - wrong! Instead, past perfect tense should preceed a past action. Eg: She had signed X when she was an adolescent . So all answer choices containing "being" is wrong.

    B) while in adolescence - wrong because the contruct "while in adolescence" is ambiguous as to "while she was in her adolescence" or "while she is in her adolescence"

this problem is almost purely a test of idioms.
more importantly, this is the kind of idiom problem that the test has essentially phased out. this problem was purged in the transition from the 10th to the 11th edition, presumably because it's no longer relevant to the test.
so, i would say you can safely ignore this one.
